Please fill out this form and mail it along with your $35.00 registration fee to:

Fax completed application to (800) 813-7114. The line will detect that there is an incoming fax and receive your document.
PAYMENT OPTIONS
OPTION A - Register for $110 (Includes $35.00 registration fee and $75.00 books and materials fee).
OPTION B - Pay the entire semester fee and save for $300.00.
OPTION C - FOR ONLINE STUDENTS - Pay entire semester fee $300.00 registration, books, and tuition fee plus $100.00 on-line fee)
The above OPTIONS apply ONLY to students pursuing course credit and the Associates Degree. If you are a M.O.S. (Monthly Observer Student) participant, you simply must pay the registration and monthly M.O.S. fee.
